CARE Survey Giveaway (“Contest”) Terms and Conditions

1. Eligibility

  • Open to U.S.-based electric and telephone cooperatives (“Cooperative”).
  • Participants do not need to be current SBS clients.
  • Limit one entry per organization, including affiliated or branch Cooperatives.
  • Filling out an application constitutes a Participant’s full and unconditional agreement to these Terms and Conditions, which are final and binding in all manner related to the Contest.

2. Entry Method

  • Applications must be completed and submitted online by the entry deadline at [website] (registration required).
  • Entry period: January 2, 2025, 8:00 am CT – March 31, 2025, 5:00 pm CT.
  • Only entries submitted by the deadline are eligible.
  • No purchase is necessary to enter.
  • Incomplete, late, or improperly submitted entries will be deemed invalid.

3. Prize Details

  • Prize: Each winning Cooperative will receive a 30-day “web-only” survey
  • Survey Content: Up to 16 questions (14 standard questions, 2 customizable single-response questions subject to SBS approval, and 1 comment box). Requests for additional customization may incur fees.
  • Survey Timing: Surveys must open and close between July 1, 2025, and September 30, 2025. Start and end times will be confirmed with SBS.
  • Survey Mechanics: Cooperative will work with SBS staff on setup and design of survey along with testing and approvals. On open date, SBS will send a broadcast email to Cooperative member database. Data must be provided to SBS following SBS database template. Two (2) business days following the completion of survey, SBS will provide Cooperative with standard results.
  • Standard Reports: Includes response percentages, daily participation tracking, and anonymized, aggregated benchmarking data which will be delivered upon completion of Contest. Winners agree to allow SBS to use aggregated, anonymized data responses for industry benchmarking.
  • Exclusions: Single sign-on, reminder communication e.g. email/SMS invites, additional reporting, printing, mailing/postage, or extra customization/programming are not included. Requests for additional services may incur fees.
  • Non-Transferable: Prizes are non-transferable, with no cash alternatives.
  • Compliance: Survey deployment must comply with SBS’ privacy and data security policies and applicable laws.

4. Winner Selection and Notification

  • Winners will be selected randomly from all eligible entries.
  • Winners will be notified by email within 10 business days of the entry deadline and must confirm acceptance within 14 business days of notification. Failure to respond may result in forfeiture, with an alternate winner selected.
  • SBS reserves the right to verify eligibility before awarding the Prize. Providing false or misleading information will result in disqualification.

5. Publicity

  • By accepting the Prize,  winners agree to allow SBS to use their organization’s name, logo, and details for promotional purposes without additional compensation.

6. Liability

  • SBS is not responsible for any injury, loss, or damage arising from participation in the giveaway or use of the prize. Participants release SBS and affiliates from liability related to Contest participation, prize acceptance, or usage.
  • SBS is not responsible for technical malfunctions, lost entries, or other errors beyond its control that may affect entry or prize delivery.

7. Legal

  • This giveaway is governed by Minnesota law and is a sweepstakes based on chance. No purchase is necessary to win.
  • SBS reserves the right to modify, suspend, or terminate the Contest or terms at any time without notice.
  • SBS’ decision on any dispute will be final and binding.

8. Privacy Policy

  • Information collected during Contest application process (e.g., name, email, cooperative affiliation, and secondary contact) will only be used for Contest administration and will not be sold or shared with third parties.
  • Personal data will be stored securely and retained only for the duration necessary to administer the Contest.

9. Additional Terms

  • Force Majeure: SBS will not be held liable for delays or cancellations due to unforeseen circumstances, including but not limited to natural disasters or technical failures.
  • Disqualification: SBS reserves the right to disqualify any participant for breach of these terms or for conduct deemed fraudulent or inappropriate.
  • Communication: By entering, participants consent to receiving communications from SBS regarding the Contest and survey deployment.
